| Project Description | WELCOME is an ambitious project which aims to bring about a change in the reactive nature of the management of chronic diseases and in particular the Chronic Obstructive Pulmonary Disease (COPD) and its comorbidities. The project will produce a patient centred and proactive approach to COPD management. It will design, develop and evaluate a ‘new ecosystem’ that will integrate the fundamental elements of care into a unified holistic system targeting COPD for early detection of complications (with the potential of reducing hospital admissions) and the prevention and mitigation of co-morbidities (CHF, Diabetes and Anxiety and Depression). | ||||
